Standard procedure forms are available to help investigators complete protocol submissions.

STANDARD PROCEDURE FORMS

IACUC protocols often include common, standard procedures.  The IACUC has created a set of Procedure Forms, containing approved language for these common procedures, that are available to you in Insight.  Instructions to add an IACUC-approved form from the Procedure Bank to your Initial Review (IR), Triennial Review (TR) or Amendment (AME) can be accessed by clicking here

You will need to customize the form by adding location and some other details before submitting to the IACUC.  You can pre-review the available forms on this website by clicking the hyperlink below. 

If you use the pre-approved procedure form, you must conduct the procedure as written so please ensure the provided language is consistent with your practice.
